Insurance and Fees

The fee per 45-minute individual session is $150, 60-minute individual session $175, 45-minute couples/family session $150, and the first intake session $200. Full payment is due at the time of our session, unless I am billing your insurance, in which case your copayment and/or deductible is due at the session.

Please let me know all the insurance plans you have that might cover my behavioral health services (ex. if you are using your EAP or have a Medicare supplemental plan). Note that I am required to provide a diagnosis on invoices and claims, and coverage may be determined by diagnostic criteria. Additionally, health plans may review treatment, limit coverage, and/or request treatment notes. While I may check coverage for you, you are responsible for verifying and understanding the limits of your coverage. While I’m happy to assist your efforts obtaining insurance reimbursement, I can’t guarantee your health plan will provide payment for services.

If I am a contracted provider with your plan: I will submit claims for reimbursement. You are responsible for paying all copayments, coinsurance amounts, and any portion of contracted fees not reimbursed by your plan. Many plans have an annual deductible (the out of pocket amount you’re responsible for before insurance begins to cover the cost of your care. Please familiarize yourself with your specific plan. If insurance does not pay as expected, you remain responsible for the balance. If I am not a contracted provider for your plan: full payment is due at each session. Upon request, I will provide you with an invoice for you to submit to your insurance company for their review and reimbursement of fees allowed under your plan.

Cancellations

Sessions are by appointment only, Monday through Friday (and some Saturday mornings). Once we book an appointment, that time is reserved solely for you. If you need to cancel or reschedule your appointment, I require a minimum of 24-hour notice (Monday-Friday business hours, and Saturday noon deadline).

Cancellations and rescheduling requests without 24-hour notice (excluding medical, family, or weather emergencies) will be assessed an $80 fee and charged to your card on file. Please note that insurance does not reimburse for missed sessions and the cancellation policy is in place to offset the loss.

I appreciate that your time is equally valuable and therefore, in the event that I cancel a session without giving you 24-hour notice, you will not be charged for your next session.

Code of Ethics

From the NASW Code of Ethics:

"The primary mission of the social work profession is to enhance human well-being and help meet the basic human needs of all people, with particular attention to the needs and empowerment of people who are vulnerable, oppressed, and living in poverty. A historic and defining feature of social work is the profession's focus on individual well-being in a social context and the well-being of society. Fundamental to social work is attention to the environmental forces that create, contribute to, and address problems in living.

Social workers promote social justice and social change with and on behalf of clients. "Clients" is used inclusively to refer to individuals, families, groups, organizations, and communities. Social workers are sensitive to cultural and ethnic diversity and strive to end discrimination, oppression, poverty, and other forms of social injustice. These activities may be in the form of direct practice, community organizing, supervision, consultation, administration, advocacy, social and political action, policy development and implementation, education, and research and evaluation. Social workers seek to enhance the capacity of people to address their own needs. Social workers also seek to promote the responsiveness of organizations, communities, and other social institutions to individuals' needs and social problems.

The mission of the social work profession is rooted in a set of core values. These core values, embraced by social workers throughout the profession's history, are the foundation of social work's unique purpose and perspective:

  • service
  • social justice
  • dignity and worth of the person
  • importance of human relationships
  • integrity
  • competence.

This constellation of core values reflects what is unique to the social work profession. Core values, and the principles that flow from them, must be balanced within the context and complexity of the human experience."
